Home and Living Assessments

Home and Living Assessments evaluate an individual’s current living environment to identify safety risks, accessibility barriers, and support needs. Our occupational therapists provide tailored recommendations—such as assistive equipment, home modifications, and personal care supports—to help individuals live more independently and confidently. These assessments are essential for NDIS planning, housing applications, and improving quality of life at home.

What is a Home and Living Assessment?

A Home and Living Assessment is a comprehensive evaluation conducted by an occupational therapist to assess how safely and independently a person can live in their current environment. It identifies barriers in the home, evaluates support needs, and recommends practical solutions such as home modifications, assistive equipment, or additional personal supports.

Who needs a Home and Living Assessment?

These assessments are beneficial for individuals with disabilities, age-related challenges, or chronic health conditions that impact daily living. They are commonly required by the NDIS to support applications for Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), or to access funding for home modifications or support workers.

What does the assessment involve?

The occupational therapist will visit your home (or conduct a virtual assessment, if needed) and review your daily routines, mobility, self-care, and home layout. They will identify areas of risk or difficulty, such as navigating stairs, using the bathroom, or entering/exiting the home. The assessment is person-centered and includes your input to ensure the recommendations suit your needs and preferences.

What is included in the final report?

The report includes a detailed summary of your functional abilities, the suitability of your current home, safety considerations, and recommendations for supports or modifications. It is often used for NDIS plan reviews or housing applications and includes clear justifications to help secure the appropriate funding and services.

How do I organise a Home and Living Assessment?

You can request a Home and Living Assessment through your NDIS Support Coordinator, Local Area Coordinator (LAC), or by contacting us directly. Once booked, we’ll guide you through the process, from the assessment to the delivery of the report, ensuring you get the support you need to live safely and independently.
